Important – CCSD requires Physical Forms & Consent Forms
to be completed on the Student & Parent Canvas Page
To fill out physical forms online, follow these steps:
- Log into student's Canvas
- Click Courses
- Click All Courses
- Click Link for Student Activities/Athletics
- On left side of screen click on link Activities/Athletics Registration
- Click link that says Start Registration
- Be sure student logs in as student and fills in the student portion
- Hand in doctor's portion of Physical to coach or Melinda O'Brien in the athletic office
